One of the core values of our practice is informed choice and full transparency. We believe that you should always have access to the information that guides your care.
This page contains the informed consent documents we use in our practice. These documents explain the benefits, risks, and alternatives related to certain conditions, tests, and care decisions that may come up during pregnancy, birth, or the postpartum period.
You are welcome to review these documents at any time throughout your care. Many clients appreciate being able to read through information in advance and return to it later if questions come up.
Please keep in mind that not every document on this page will apply to every client. Some consents relate to specific circumstances or conditions that may never arise in your care. We keep them all available here so that you can see exactly how we counsel and document informed decision-making when those situations do occur.
If a situation arises that requires one of these consents, your midwife will review the information with you in detail and answer any questions before anything is signed.

General Pregnancy & Routine Care
TDLR Informed Choice & Disclosure Agreement - This is a document issued by the Texas Department of Licensing and Regulation (our governing body) that explains details about our licensure as well as things we are required to to as well as prohibited from doing.
Routine & Optional Testing in Pregnancy - This is a document that outlines all routine labs as well as additional ones you may opt to have done during your pregnancy. It also includes routine sonograms.
Student Involvement Agreement - This document gives you the ability to chose how involved in your care our students are.
Required Recommended Referrals - There are certain conditions we as midwives are required to recommend you be seen by a physician or other care provider to be assessed for. This form covers all of the recommended referrals as laid out by TDLR rules and regulations which can also be found here.
Informed Choice Refusal Form - This is a form we keep on hand for situations that may occur during your care that we do not have a dedicated consent form for. In an circumstances we use this form we will have a thorough discussion about any risks and benefits to our recommendation as well as any alternatives that will all be documented on the form before you sign it.
Labor & Birth Considerations
Group Beta Strep bacteria (GBS) - GBS is a common bacteria found in our gut. About 1 in 4 pregnant women will test positive for GBS bacteria. We routinely screen for GBS bacteria with your initial labs through a urine culture as well as around 36 weeks by vaginal/rectal swab.
Placenta - Everyone regardless of place of birth has the option to keep your placenta and decide what to do with it. If you wish to keep your placenta, be it to encapsulate, bury under a tree in your yard, and anything in between you will be required to sign a placenta release form created by the Texas Department of State Health Services if you give birth in the birth center. If you give birth at home you will not be required to complete this form.
Pregnancy Conditions That May Arise
Advanced Maternal Age - ACOG defines advanced maternal age as any pregnancy in which the mother will be 35 years of age or older at the time of birth.
Gestational Diabetes Mellitus (GDM) - Everyone is screened for gestational diabetes during their pregnancy between 24-28weeks. Gestational diabetes typically affects up to 9% of pregnancy in the United States each year.
Vaginal Birth After Cesarean (VBAC) - We support mothers in their right to chose a vaginal birth after cesarean.
Breech Delivery - We do offer breech delivery on a case by case basis. If your baby is breech we will have an in depth discussion about your options.
Gentle External Cephalic Version (ECV) - Some mothers who have a baby in the persistently breech position may opt for an ECV.
Twin Delivery - As with breech delivery we will attend twin births on a case by case basis with thorough discussion of risks and benefits.
Postpartum & Newborn Care
Vitamin K administration - The standard of care for all newborns is to administer an injection of vitamin K after birth. You as the parent of your child have a say in if/how this is done.
Erythromycin administration - The state of Texas has a legal requirement for every baby be administered erythromycin antibiotic ointment within 2 hours of birth. As the parent of your child you have the right to refuse this. You will only be required to complete this form if you are declining erythromycin administration for your baby.
NBS/CCHD/TEHDI - This consent form covers several different tests that are required and recommended for your baby within the first 8 weeks of life. Some of them we perform in house during your routine postpartum visits, while some you will need to arrange to have done separately with a different provider.
Immtrac - Immtrac is an immunization tracking system provided by the state of Texas to keep track of your child’s immunizations through a central tracking service. You will only complete this form if you consent to having your baby enrolled in the Immtrac registry.
